Referrals
We welcome referrals from professionals, individuals, and family members who are seeking support. Our referral process is designed to be clear, straightforward, and centred around individual needs.
Our Referral Proccess
Upon receipt of an initial referral from a Care Manager or other relevant professional, Home From Home will review the information provided and arrange a full assessment of the individual where appropriate.
The assessment process is designed to ensure that all relevant information about the individual’s needs, preferences, and aspirations is clearly identified. This enables us to determine whether we are able to provide the care and support required. Throughout the assessment, we will speak with the individual, the referrer, and any other relevant parties to ensure the process is thorough, accurate, and person centred. All assessments are carried out by experienced and appropriately trained senior members of our team. Consent will always be sought from the individual before accessing information held by other agencies.
If it is determined that Home From Home is able to meet the individual’s needs, a proposed care and support package, including associated costs, will be provided in writing to the Care Manager and or referrer. Where the proposal is agreed by all parties, a service delivery plan and relevant risk management plans will be developed prior to the commencement of the service.
Once support begins, the initial personal plan is reviewed and updated within the first seven days, in line with the outcomes of the assessment. The personal plan is clear, accessible, and designed to guide staff in providing daily care and support that meets the individual’s needs and supports their personal outcomes. Plans are developed and reviewed in consultation with the individual receiving support and, where appropriate, their representative.
