Home From Home provides a personalised supported living service for adults with mental health needs, learning disabilities and or autism, enabling people to live safely and as independently as possible within their own homes across South Wales.
What is Supported Living?
Supported living enables adults to live within their own homes while receiving tailored support to meet assessed needs. Support is delivered on behalf of local authorities and health boards and is designed to promote independence, stability and meaningful participation in everyday life.

















Our Approach to Supported Living
Our supported living approach is recovery-focused and person-centred, promoting choice, dignity, stability and meaningful participation in everyday life. Support is tailored to assessed needs and agreed outcomes, ranging from targeted daily support to 24-hour provision where required.
Our skilled workforce supports individuals to develop life skills, maintain their accommodation, strengthen emotional wellbeing and engage positively within their communities. We work collaboratively with health professionals, families and community services to promote continuity, stability and positive outcomes.
Operating across multiple local authorities within the Gwent footprint, we deliver commissioned services on behalf of local authorities and health boards. Our services are delivered within robust governance and safeguarding frameworks, with ongoing quality monitoring to ensure support remains consistent, safe and outcome focused.

Regulation and Quality
Home From Home Support’s supported living service is regulated by Care Inspectorate Wales and delivered in line with Social Care Wales standards.
We operate within robust governance, safeguarding, and quality assurance frameworks, working in close partnership with local authorities, health boards, families, and other professionals to ensure support is safe, consistent, and outcome focused. Our service delivery is continuously monitored and reviewed to ensure we meet commissioning requirements and maintain high standards of care.
Our commitment is to provide stable, therapeutic, and person-centred support that enables individuals to live safely, confidently, and independently within their own homes and communities.
Referrals
Referrals to our supported living service are carefully reviewed to ensure we are able to meet the individual’s assessed needs and provide the right level of support within their home or supported living setting. Where appropriate, a comprehensive assessment is carried out to understand personal preferences, aspirations, risks, and support requirements.
If it is determined that we are able to provide the service required, a proposed support package is developed and shared with the referrer or commissioning body for agreement prior to the commencement of support. Individual support plans and associated risk management plans are developed in consultation with the person receiving support and, where appropriate, their representative, and are reviewed regularly to ensure they remain responsive and outcome focused.

Home From Home delivers supported living services within individuals’ own homes or commissioned supported living schemes. We do not operate as a landlord. Our role is to provide personalised care and support that enables people to live as independently as possible within their accommodation.

We provide supported living services across South Wales, operating within multiple local authorities across the Gwent footprint, including areas such as Blackwood and surrounding communities. Services are delivered on behalf of local authorities and health boards.
