Supporting independence at home
Home From Home delivers a flexible, person-centred domiciliary care service providing targeted, community-based support. We support adults with diverse and often complex needs, including mental health conditions, autism, learning disabilities, and physical health needs, to live safely and independently within their own homes across several local authorities.
Delivered as a commissioned service on behalf of local authorities and Health boards, the service enables people to remain safe, independent, and emotionally well within their own communities, while retaining choice, dignity, and control over their daily lives.
Support is tailored to assessed needs and agreed outcomes and may include medication management, personal care, domestic tasks, emotional reassurance, attendance at appointments, and supported access to the community. Support is proportionate and responsive, with support levels adjusted in line with changing needs.

Regulation and Quality
Home From Home Support’s domiciliary care service is regulated by Care Inspectorate Wales and delivered in line with Social Care Wales standards.
We operate within robust governance and safeguarding frameworks and work in close partnership with individuals, families, professionals, and commissioning bodies to ensure care is delivered safely, responsibly, and consistently. Quality is continuously monitored and reviewed to ensure our services remain effective, responsive, and focused on positive outcomes.

Referrals
Referrals to our domiciliary care service are carefully reviewed to ensure we are able to meet the individual’s needs. Where appropriate, a full assessment is carried out to understand personal preferences, aspirations, and support requirements.
If we are able to provide support, a proposed care and support package is shared with the referrer and agreed prior to the commencement of the service. Support plans are reviewed regularly and developed in consultation with the individual and, where appropriate, their representative.
